Who is a SAP (Substance Abuse Professional) and what exactly is SAP evaluation?
Substance Abuse Professional (SAP) services a transportation worker who has violated DOT regulations (DOT Regulations, 49 cfr part 40) must follow a specific procedure before being considered for return to safety-sensitive process, conducted by a qualified Substance Abuse Professional (SAP).
The SAP’s function is to protect the public interest in safety by professionally evaluating the employee and recommending appropriate treatment/education, follow-up testing and aftercare.
The primary safety objective of the DOT rules is to prevent (through deterrence and detection) alcohol and controlled substance users from performing transportation industry safety-sensitive functions.
What to Expect:
THE DOT SAP EVALUATION IS A MULTI-STEP PROCESS: At your first appointment, the SAP conducts an initial face-to-face clinical evaluation, including standardized testing. We will discuss the reason for the substance abuse evaluation, previous substance abuse issues, addiction treatment, DUI, mental health, drug and alcohol history, family, social, physical health…. The evaluation is comprehensive and ususally lasts three hours, followed by a random drug/alcohol test. The SAP will then write your evaluation report and make treatment recommendations based on this interview, objective data, your self-report and any collateral contacts, letters, 12 step meeting documentation. Pleaase bring any and all documentation to support your case.
SAP Evaluation contains treatment recommedation: DOT's rule requires recommendation for treatment and/or education in every case. This report and recommedation will be sent to identified parties. An initial evaluation report is sent to the Designated Employer Representative (DER). The SAP can then assist you in locating the appropriate treatment provider(s). The SAP may assist you in referrals to specific treatment provider(s).
The DOT SAP evaluator does not provide treatment services or psychotherapy, the SAP serves as a guide, liaison and evaluator. The treatment facility or psychotherapist should maintain contact with the SAP and update SAP on client progress.
After you have completed all of the SAP treatment recommendations, call to schedule a face-to-face follow-up clinical evaluation. The SAP will determine if the employee has complied with the SAP’s recommendations. If the employee has complied with the SAP's recommendation, the SAP sends a follow-up report is to the DER, including SAP’s recommendations for a program of aftercare and follow-up testing*.
*Note that random drug and alcohol testing recommendations are also required in this follow-up report per DOT rule and continue for period of time even after “return to duty”.
